Supporting a successful transition

Planning and preparing for the transition between phases of education and preparation for adult life should form part of the special educational needs support the child or young person receives.

The school should share information with the new school, college or other setting to enable a successful transition. Any information shared should be discussed and agreed with the child’s parents and/ or young person.

Sometimes the young person may be staying at the same school post-16. If this is the case, the best way to provide a high quality study programme should still be considered.

Secondary school transfer

If your child is currently in year 5, has a statement of SEN or an education, health and care plan (EHCP) and will be leaving primary school in summer 2016, you will need to have applied for a secondary school place by the end of July 2015.
