Preparing for Adulthood Team

The City of York’s Multi-Agency Team supporting disabled young people and those with Special Educational Needs as they become adults

The Preparing for Adulthood Team begins by finding out about the young person’s hopes and aspirations. Working in partnership with young people and their families the team provide support to:

  • access individual budgets 
  • find an appropriate course or work experience. 
  • access training to increase independence travel skills 
  • develop plans to live in their own home

Children's Social Care: Social Workers

Social workers work with young people aged 14-18 who have physical and learning difficulties or complex health needs.

Adult Social Care: Adult Care Managers

Care managers jointly work with a children’s social worker, a young person and their family from 16 years until they are settled in adulthood.

York Independent Living and Travel Skills  (YILTS) supports young people to gain skills and confidence in travelling independently. YILTS works with young people aged 11–19.

Specialist Careers Advisers

This team provide impartial information, advice and guidance and personal development opportunities to young people and families to ensure a smooth transition to adulthood. Every young person with an Education Health and Care plan from year 9 until the EHCP ceases has a named Adviser.
